Please complete and submit all necessary documents before your first visit to Creature Comforts for a quick and smooth check-in.  Please be as thorough as possible in answering all of the questions, this information is vital to our relationship with your pet and his/her care. All pets who visit Creature Comforts must be current on all vaccinations, including Bordetella (not a regularly given vaccine),  parasite control; and if participating in group play, all pets must be spayed or neutered. We ask that you submit current certificates from your veterinarian each year upon vaccination if you are planning a stay with us. Those vaccination records can be emailed to info@CreatureComfortsBoarding.com, or faxed to 479-289-5352, or you can bring them by.

The documents below are provided in E-Forms to fill both forms out online and submit to us.

*Please note that when making an online reservation request that we will reserve the size room that you normally request, or the size that is available for the dates requested. You may put in the comment space the room size that you prefer and we will try to reserve that size, if it’s available.

For your convenience, all forms are also available at the desk when you visit Creature Comforts. Don't forget to have your veterinarian fill out vaccination records each year and send them to us!
